Emergency services rushed to the scene on the New England Highway, about 30km south of Uralla, about 4.30pm after reports of a serious collision.
The Westpac Rescue Helicopter Service tasked its Newcastle and Tamworth-based choppers to the accident after initial reports suggested three people had been injured, but the Hunter helicopter was called off en-route.
A spokesperson for NSW Police confirmed two people had been killed at the scene.
Police said a man behind the wheel of a Subaru, believed to be aged in his 20s, died at the scene.
The male driver of the other vehicle, a Toyota, died at the scene. He is believed to be aged in his 40s.
Ambulance paramedics treated a 25-year-old man, a passenger in the Subaru, who was trapped in the wreckage.
It’s believed he suffered leg and pelvic injuries and was transported to Tamworth hospital by road ambulance in a serious condition.
The crash shut the highway in both directions for several hours as New England detectives and crime scene officers combed the road to try and determine what caused the crash.
Traffic was diverted around the scene via Walcha on the Oxley Highway and Thunderbolts Way.
A report will now be prepared for the coroner.
DIVERSIONS IN PLACE
The Westpac Rescue Helicopter has tasked both its Newcastle and Tamworth-based choppers to the crash amid reports two people are still trapped in the vehicles.
It’s understood one person has been killed in the crash.
The RMS has shut the highway at Uralla and at Bendemeer with all traffic being turned around.
Diversions have been put in place via the Oxley Highway at Bendemeer and Thunderbolts Way at Uralla, through Walcha.
Motorists are being urged to use caution with delays expected.
The road is expected to remain closed for several hours.

POLICE have shut the New England Highway between Uralla and Tamworth after a head-on collision on Wednesday afternoon.
Emergency services were called to reports of a crash between a vehicle and a small truck about 30km south of Uralla, at about 4.30pm.
It’s understood there are multiple patients.
Police have shut the highway and traffic is being turned around.
