Police allege the teacher committed sex offences as a 27-year-old against a 15-year-old boy at a high school in Birrong in 1997.
It's also alleged she sexually assaulted another 15-year-old boy at the same school that year.
Officers from the Bankstown Local Area Command launched an investigation into the alleged sex offences in February, and at 7am on Monday they arrested a woman, now aged 47, at her Padstow home.
The woman, who is still a teacher, was charged with 16 counts of aggravated sexual assault and five counts of act of indecency.
She was granted conditional bail and is due to appear in Bankstown Local Court on January 10.
Police are encouraging anyone who may have been a victim of sexual assault to come forward, regardless of how long ago the incident happened. Reports can be made at any police station.
